Mitzi Cardona has been with the American Heart Association since 2017, recently as the National Program Lead – Collegiate Diversity Partnerships – Hispanic Serving Institutions (HSIs). Mrs. Cardona has been instrumental in building and sustaining strong partnerships with HSIs and has fostered relationships with 90 HSI scholars over the last two years. She has been involved and engaged with FQHCs, Health Systems, local Community-based Organizations and Collective Impact Groups. Through her partnerships, she has helped established sustainable health outcome changes for Hispanic/Underserved communities by developing and implementing patient and clinical educational programs to help them-apply and practice the most up-to-date, evidence-based treatment guidelines for High Blood Pressure, Cholesterol, Diabetes and addressing Social Determinants of Health.
In addition to her responsibilities, Mrs. Cardona serves on three local board of directors, Covenant Community Care Health Center, The Information Center, and Christ to the Nations Church. Among many awards most recently Mrs. Cardona received the Community Health Champion Award from Western Wayne Family Health Centers. Mrs. Cardona attended University of Puerto Rico- Arecibo, Majored in Social Sciences. Has been married for 25 years and has two children.
